To start residential service with the Oskaloosa Municipal Water Department, please download, complete, and print our application for service. Please include a copy of your photo ID with your application along with your deposit of $100.00.
Completed forms may be brought into the office at 1208 South 7th Street during regular business hours, e-mailed to customer.service@oskaloosawater.org or mailed to:
